District Nurses are part of your wider primary healthcare team and are provided by Dorset Healthcare University Foundation Trust, one of our trusted NHS partners.
They support patients who need nursing care at home, particularly those who are housebound or have complex health needs.
District Nurses can help with:
Wound care and dressings
Injections and medicines support
Catheter and continence care
End-of-life and palliative care
Ongoing support for long-term conditions
Preventing unnecessary hospital admissions
Care is tailored to each patient’s needs and provided with dignity, compassion, and professionalism.
Referrals are usually made by a GP, nurse, or hospital team
The service works closely with our practice to ensure joined-up care
District Nurses play a vital role in helping patients stay safe and supported at home.
