For many people, circumstances beyond the purely medical gives rise to additional worries, issues and needs.  Our Social Prescribing and Wellbeing Team work with our patients giving them time to focus on “what matters to me” and take a holistic approach to their health and wellbeing, by offering help to access community groups and statutory services for practical and emotional support.  Helping to improve their health, wellbeing and social welfare.

Wellbeing schemes can involve a variety of activities which are typically provided by voluntary and community sector organisations. Examples include volunteering, arts activities, group learning, gardening, befriending, cookery, healthy eating advice and a range of sports.

The Team

Portesham Surgery is pleased to confirm that we are now one of many surgeries across the region offering Social Prescribing and Wellbeing Services to their patients.

The Team are passionate about supporting our communities in a non-clinical way.  The Social Prescribers and Care Co-ordinators will work with patients who are lonely, have complex social needs, low level mental health needs and long term conditions.  They will support them, connecting them to non-medical community-based activities with the help of our Health Champions, meeting practical, social and emotional needs including physical activities, arts and nature.

Do you look after someone who couldn't manage without your support?

If the answer is yes, then you are a carer.  

You may not consider yourself a carer, as caring is often something we do for family or friends without question and without defining or labelling it as such. If you provide care to someone who needs help because of their illness, frailty, disability, a mental health problem or an addiction, and they couldn’t manage without your support, then you are a carer.

It is really important that your Surgery knows that you are a carer, as this will give you access to additional support.  Please click the link above and then click on the Carer's Registration Form to let us know that you are a carer - this information will come through to Portesham Surgery so our Carer's Lead can contact you and give you any support that you need.
